An Occupational therapist will complete a home visit to determine equipment needs, personal care needs, and housekeeping needs of an individual after an injury or illness and provide recommendations for improved safety and optimal functioning within their homes.
If needed, the Occupational therapist will work with the homeowner and contractor to develop a home modification plan for an optimal functional and aesthetically acceptable design for the home renovation.
Our Occupational therapist will complete an in-home evaluation of an individual's ability to carry out everyday tasks to determine their eligibility and need for attendant care services following injury or illness. Our assessments consist of a clinical interview with the client; collateral information from the client's family, friends and medical rehabilitation team; relevant physical, cognitive and psychosocial screening/evaluation; documentation review, and observations of the client's home.
For an individual experiencing pain symptoms, which persist 3+ months, a Nurse Practitioner will evaluate various physical, cognitive and psychological factors of their injury or illness and produce a report with a history of their condition, as well as make recommendations for addressing their pain symptoms.
An analysis of the environment and the individual's work style to identify risk factors that may result from high repetition and awkward postures in any environment that an individual may work. The OT will provide a report that will outline a program for safe work practices and any modification or equipment needed.
An OT or kinesiologist performs standardized tests and measurements of an individual's musculoskeletal system to address ROM, strength and endurance. Through this assessment, the degree of injury or limitation which impacts function can be measured and used to direct the rehabilitation process.
An Occupational Therapist will assess the needs of the individual before discharge from the hospital or care facility and perform an in-home assessment to recommend adaptive equipment, modifications and support care as needed. This assessment makes the transition from hospital to home safe and supported.
A certified assessor will complete an assessment of an individual's need for assistive devices (such as wheelchairs, hearing aids, etc.) and, if appropriate, will complete a report and application for funding from the ADP program.
This is used to discover if injuries following a motor vehicle accident (MVA) fall within the Minor Injury Guideline of auto-insurers policy. This assessment consists of a history of the client, physical examination, reviewing other health records, and conferring with other members of the client's clinical team. It may require further assessment with the Nurse Practitioner to confirm a thorough diagnosis following an MVA. This assessment is used to access increased funding for rehabilitation through an auto insurance policy, if appropriate.
This assessment is performed by a registered dietician to provide nutrition care needs to individuals and assist in managing illness and injury recovery.

A comprehensive assessment that measures an individual's current functional abilities and physical tolerances. This assessment provides objective information about an individual's ability to perform work-related tasks and identify transferrable physical skills.
