A Hospital Discharge Assessment is a critical service that ensures a safe and smooth transition from hospital to home following an injury, surgery, or serious illness. Conducted by an Occupational Therapist, this assessment identifies the supports, equipment, and services a patient will need to return home safely and continue their recovery.
The assessment typically takes place before discharge and involves a review of the patient’s medical condition, functional abilities, home environment, and potential risks. The Occupational Therapist evaluates the individual’s ability to perform essential daily activities such as mobility, bathing, dressing, toileting, and meal preparation.
Based on the findings, the therapist provides detailed recommendations that may include assistive devices, home modifications, personal care support, or follow-up rehabilitation services. If needed, the therapist can also coordinate with hospital staff, family members, and community resources to ensure the necessary supports are in place.
