Job Description
Vitality Rehabilitation Group is a leading healthcare organization dedicated to providing comprehensive rehabilitation services to our patients. We are currently seeking a highly motivated and compassionate Social Worker (MSW) to join our team. As a Social Worker at Vitality Rehabilitation Group, you will play a vital role in supporting the emotional, social, and psychological needs of our patients and their families.
Responsibilities:
- Conduct thorough assessments of patients’ psychosocial needs, including emotional, social, and financial aspects
- Collaborate with the interdisciplinary team to develop individualized care plans
- Provide counselling and support to patients and families, addressing issues such as coping mechanisms, adjustment to illness or injury, and end-of-life planning
- Assist patients and families in accessing community resources and support services
- Facilitate patient and family education to promote understanding and engagement in the treatment process
- Advocate for patients’ rights and ensure their needs are met within the healthcare system
- Maintain accurate and confidential documentation of patient interactions and progress
- Participate in interdisciplinary team meetings and contribute to the overall patient care plan
Qualifications:
- Master’s Degree in Social Work from an accredited program
- Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum of 2 years of experience in a healthcare or clinical setting, preferably in rehabilitation
- Strong knowledge of medical terminology, patient assessment, and treatment planning
- Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and families
- Ability to work effectively within an interdisciplinary team and collaborate with healthcare professionals
- Proficient in conducting psychosocial assessments and providing evidence-based counseling interventions
- Familiarity with community resources and support services for patients and families
- Exceptional organizational skills and attention to detail
- Ability to maintain confidentiality and adhere to ethical guidelines and professional standards
Job Type: Part-time
Range of Pay: $35.00-$55.00 per hour
